NHS patients have a legal right choose their first outpatient appointment. However, some GPs do not understand how it works. Siobhan Rourke discovered this when she attempted to get an ADHD assessment via Right To Choose.

Right to Choose is the legal right that NHS England gives patients to select their own team of doctors to provide non-urgent care that is supervised by consultants. This includes psychiatry. However, private psychiatrist cost uk doesn't apply to services provided by local authorities such as maternity services or community mental health services.
To avail the right to choose service it is necessary to be referred to by your GP. This is a GP that is registered with the General Medical Council (GMC). Go to the NHS website to see which GPs have been approved by the GMC.

This right of choice is only applicable to services that have a standard NHS contract with your CCG, or NHS England for the kind of service you need. This includes psychiatry.

To understand how GPs make referral decisions the TAG team conducted a series of facilitated focus group sessions with GPs and MHPs at eight different GP practices. The conversations were recorded and transcribed verbatim. In general, doctors were not inclined to refer patients for medication or psychotherapy. They were concerned that patients wouldn't be able to handle the additional treatment, and that they might not attend the follow-up appointments. Many GPs also believed that liaison psychiatry had inadequate resources and was oversubscribed.
The results of this study revealed that GPs relied on their pre-existing information and relationships to guide their referral decisions more than the specific needs of patients. The recorded meetings showed this. CMHT leaders, Psychiatrists, and GPs all expressed their opinions on the competence of GPs. This was evident in every recorded meeting.
It is important to keep in mind that the right of choice applies to both treatments and assessments including neurodevelopmental diagnostic assessments. The current pandemic has caused an enormous increase in the number of people being referred to mental health professionals within their communities for help. This has resulted in an increased need for psychiatry assessments as well as an increase in the workload of staff. It was challenging for GPs to keep up with referrals due to the rapid spread of the pandemic. This led to a large number of inappropriate referrals.
In response to this, we conducted an qualitative study nested within a randomised controlled trial to determine the effectiveness of a range of strategies aimed at improving the quality of referrals for mental health. This included interviews with GPs, community health team (CMHT) consultants, and the recording of referral allocation meetings. The study concluded that a bespoke pathway for referrals was a viable method of managing the workloads of GPs and CMHTs while giving patients with the flexibility needed to receive the best treatment.
